Long term conditions– annual reviews:

We are continuing to streamline our process for our patients that need an annual review (planned care review). An e-consult form may be sent to you with you with your invite. Completing the template ahead of you planned care review, will enable to doctor to construct/ develop your care plan. It also allows for the review process to be as smooth as possible. The clinical team will be able to easily update yourrecords so that you have more time doing your appointment to discuss your health conditions.

Depending on the health conditions being reviewed, there may be multiple steps involved. Different clinicians are responsible for specific parts of the review, so for some conditions, you will need more than one appointment. This may be either face to face or by telephone. If you have a preference for the clinician that you would like to carry out your annual review, please do let us know and we will try our best to accommodate. If you have any questions about your annual review, please get in touch in the surgery and speak to a member of the planned care team.

From our pharmacy team

As the clocks change and we approach summer, we also unfortunately approach hay fever season. Symptoms include; continuous sneezing (especially first thing in the morning), runny or blocked nose, itchy nose and eyes, watery eyes, sore and scratching throat cough and headaches. If you experience any of these symptoms, please speak to you community pharmacist who can recommend over the counter antihistamines and hay fever products to help ease these symptoms. Cetirizine and Toratadine are ‘non-sedating’ antihistamines, which are less likely to cause you to feel drowsy than other antihistamines and can be taken daily. If you do not see any improvement in your symptoms within 2 to 4 weeks, please speak to your chemist for further advice.
